Steps

Go to Layer > New Adjustment Layer > Color Balance and match your settings to mine:


Make a new raster layer and fill it with the color #D5F03F and set it to Overlay at 37% opacity.
Go to Layer > New Adjustment Layer > Channel Mixer and match your settings to mine:




Go to Layer > New Adjustment Layer > Hue/Saturation and match your settings to mine:


Make a new raster layer. Fill it with the color #7CD2E1 and set it to Overlay at 28% opacity.

Go to Layer > New Adjustment Layer > Hue/Saturation and match your settings to mine:


Now we're going to add the text. Start out with any font (I'm using Paulinho Pedra Azul) using the color #314448. Set your text layer to Vivid Light. Right click your layer and go to Blending Options and match your settings to mine:

Color used #ffffbe


And viola! You should be all done :)
